About the Role:
We are looking for a dedicated and skilled Consultant Pharmacist to join our remote team and provide exceptional medication management services across the Wisconsin territory. In this role, you will be instrumental in enhancing facility compliance, fostering prescriber engagement, and optimizing medication therapy to improve the quality of life for residents. This position offers the flexibility of remote work with a focus on delivering top-notch clinical support and medication management to healthcare facilities.
Key Responsibilities:
Medication Evaluation: Assess physician medication orders for appropriateness, including drug choice, dosage, potential interactions, and stability. Provide recommendations for therapy adjustments when needed, in collaboration with physicians and facility staff.
Resident Care: Conduct monthly reviews of each assigned resident's drug regimen and clinical indicators, reporting any concerns to the Director of Nursing, Administrator, and attending physicians with written findings and recommendations.
Documentation: Maintain clear and detailed records of all communications with physicians, nurses, healthcare teams, residents, and their families.
Regulatory Compliance: Stay current on pharmacy laws and federal/state regulations for long-term care, ensuring all practices are compliant.
Collaboration: Work cooperatively as part of a multidisciplinary team, offering support and guidance to care teams, including nursing staff and prescribers.
Clinical Expertise: Apply your knowledge of pharmaceutical care practices specific to long-term care to improve resident outcomes and ensure medication therapies are optimized.
What We Are Looking For:
A Bachelor's degree in Pharmacy or a Pharmacy Doctorate from an accredited institution.
A current pharmacist license; dual licensure is preferred.
At least one year of experience in acute, long-term care, or home health settings is preferred.
Previous experience as a Consultant or Clinical Pharmacist is a plus.
IV clinical experience is preferred but not required.
Must be willing to work weekends or outside of normal business hours when necessary.
Immunization certification through an accredited organization (e.g., APhA) is required, with the ability to obtain it within 90 days of hire.
Ability to lift patient charts and stand for extended periods when needed.
Strong customer service skills and a commitment to providing the highest quality care.
