Summary of Services:
The Medical Director will provide services for Walworth Countys Lakeland Health Care Center which is a 5Star postacute rehabilitation skilled nursing facility licensed by the state of Wisconsin. We accept Medicare Medicaid private pay and most insurances. The purpose of Lakeland Health Care Center is to provide high quality affordable care to the citizens of Walworth County.
Mandatory Qualifications:
- Valid State of Wisconsin general geriatric or similar medical license with no suspensions or restrictions.
- Have current unrestricted federal and medical licenses or permits to prescribe controlled substances which must remain in effect during the term of the resulting contract.
- Fully credentialed for Medicare and Medicaid Services.
- Possess an American Board of PostAcute and LongTerm Care Medicine (ABPLM) Certified Medical Director (CMD) Certification.
- Possess a minimum of one (1) year of verifiable experience working in a longterm care setting or providing medical director services.
- Be able to commit a minimum of twenty (20) hours per month on average for medical director services to be provided to Lakeland Health Care Center.
Duties/Responsibilities:
- Conduct an initial examination of each new admission to Lakeland Heath Care Center within 72 hours of admission. Follow up visits by the Medical Director are dependent upon the admission date of the resident.
- Provide attending physician services. Each resident has a primary physician assigned to them.
- Be responsive to Lakeland Health Care Center and involved in providing feedback regarding policy and procedure changes and updates to fit current best practices.
- Be available via phone or email to assist during Department of Health Services Department of Quality Assurance (DQA) annual recertification survey as well as necessary during any additional state investigation of Lakeland Health Care Center.
- Show interest in partnering with and providing ample time examining residents.
- Ensure all physicians are completing their monthly tasks signing orders etc.
- Meet with clinical management at Lakeland Health Care Center to discuss terms for care delivery communication and policies and procedures for any updates or concerns.
- Must attend the monthly QAPI (quality assurance performance improvement) committee meetings.
Medical Staff:
- In collaboration with the individual physician to act as Medical Director must develop and implement resident care policies that are consistent with current professional standards of practice for not only pain management and symptom control but for assessing residents physical intellectual emotional social and spiritual needs as appropriate.
- These policies must be reviewed and approved at a minimum annually or as needs arise.
- Physicians are held accountable to meet requirements for physician visits infection control decrease in unplanned hospital transfers and clinical capabilities of our community at Lakeland Health Care Center.
- Lakeland Health Care Centers staff will work with the individual physician to act as Medical Director to ensure the appropriate and latest information is communicated to all caring for residents.
- Provide information regarding a backup plan in the event that the main physician assigned as Medical Director is not available.
- After the initial visit of a new admission the resident must be seen by a physician at least once every thirty (30) days for the first ninety (90) days of their residency at Lakeland Health Care Center. These visits must be completed by a physician.
- After the initial visit and the first ninety (90) days the resident must be seen at least once every sixty (60) days thereafter. The physician or their designee (including a nurse practitioner or a physicians assistant) may complete these and every other required visit for the duration of the residents care.
- Progress notes are required on all patient visits and are required to be sent to Lakeland Health Care Centers medical records department if not entered directly into the electronic medical record Point Click Care (PCC) by the Medical Director or their designee.
Nursing Staff:
- Nursing Staff leadership with input from the individual physician to act as Medical Director shall delineate the competencies required for all nursing staff to deliver individualize and provide safe care for Lakeland Health Care Centers residents.
- There must also be a process to evaluate staff skill levels and to develop individualized competencybased training that ensures resident safety and quality of care and service being delivered.
- Communication with Lakeland Health Care Centers Administrator and/or Director of Nursing (DON) on all concerns updates changes planned/unplanned absences and any other open communication is crucial and required.
A CompetencyBased Program Shall Include the Following Elements:
- Evaluates current staff training programming to ensure nursing competencies (e.g. skills fairs training topics return demonstration).
- Identifies gaps in education that are contributing to poor outcomes (e.g. potentially preventable rehospitalization) and recommends educational programing to address these gaps.
- Outlines what education is needed based on the resident population (e.g. geriatric assessment mental health needs) with delineation of licensed nursing staff versus nonlicensed nursing and other staff member of Lakeland Health Care Center.
- Delineates what specific training is needed based on Lakeland Health Care Centers assessment (e.g. ventilator IVs Tracheostomy).
- Details the tracking system or mechanism in place to ensure that the competencybased staffing model is assessing planning implementing and evaluating effectiveness of training.
- Ensures that competencybased training is not limited to online computer based but should also test critical thinking skills as well as the ability to manage care in complex environments with multiple interruptions.
- The Medical Director or their designee is required to be on call for Lakeland Health Care Center 24/7 in case of emergency.
- The Medical Director or their designee will be responsible for all coding and billing of medical services provided within Lakeland Health Care Center.
- The Medical Director or their designee will submit billable medical services to applicable insurances for reimbursement and retention of all revenues related to services provided within Lakeland Health Care Center.
- Ensure any other staff providing services has all necessary licenses and credentials as outlined herein and required by all regulatory agencies to perform associated services.
